Meant for individuals who can work at their own pace at a time that works for them rather than a strict classroom schedule.

Nurses can provide medical advice on what potential options are available such as virtual care, primary care, urgent care options, or seek emergency care if needed at a Sanford Health Systems location.

Offers drop-in centers for individuals living with a mental illness to gather, be with peers and friends, chat, and have some fun. Socialization activities are available at the centers.

Provides short-term respite or rehabilitation care in home-like settings. Services include: - 24-hour care services- Community and enrichment programming- Dementia and Parkinson's services- Family member support and communication- Fully furnished private bedroom- Home-cooked meals and daily snacks- In-home occupational and physical therapy- Medication management

The availability of financial assistance/vouchers varies month by month and may be limited to partial payments depending on the amount owed and funds available. Financial emergency assistance may be available once every 12 months and considered on a case-by-case basis for the following services:- Food vouchers- Furniture vouchers and referrals to Bridging to receive furniture- Gas money vouchers or gift cards are available once every 6 months, depending upon the situation- HeatShare program provides fuel oil, gas, and electric payment assistance to avoid disconnection- Information and referral for eye care and food shelf - Networking support- Other financial assistance for emergencies beyond the individual's control- Pathway of Hope: Mentoring program for families. Provides short-term grief support counseling for youth and adults and their families who are grieving the death or terminal diagnosis of a family member. Also provides referrals to mental health providers who offer long-term grief support, trauma therapy, or psychiatric care, and will assist clients with referrals to legal resources.

Plans and develops with local partners a variety of home and community-based services for older people so that older people can stay in their own homes and communities- Consults on a wide range of topics from demographic changes and needs of older people to programs and services for a variety of organizations, individuals, and businesses- Leverages resources and grant or contract funds to local providers- Provides demographic and service need analysis that informs policy makers, the public and media- Provides TA to Age-Friendly communities, a concept so that communities in the region are responsive and sensitive to the needs of older adults and their caregivers- Provides technical assistance for local service development projects, including state Live Well at Home funds under the Eldercare Development Project

The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.

Services include:- Adult Rehabilitative Mental Health Services (ARMHS): Holistic approach that places the person at the center of all interventions. The goal is to restore each person's ability for independent living, socialization and effective life management. ARMHS instructs, assists, and supports the person in areas such as medication, education, and monitoring basic social and living skills. Assistance is also provided with mental illness symptom management, household management, employment support and transition to community living.- Assessments and evaluations for mental health. Diagnostic assessments and psychological testing for all ages.- Case management services- Children's Therapeutic Services and Supports (CTSS) to provide psychotherapy and skills training- Crisis intervention- Mental health counseling for adults, children, and families. Services are based on individual, child, and family needs.
