Services provided are: Case Management - Assessing barriers that prevent stable housing - Crisis intervention - Connections to resources for housing - Provides budgeting, life skills support toward employment and education goals Coordinated Entry - Assessments for supportive housing programs and other support services - Fond Du Lac Property Management provides 24 supportive housing units through coordinated entry - Fond Du Lac Property Management provides 10 veteran supportive housing units through coordinated entry Emergency Rental Assistance Program - Assist with one month's rent and security deposit not to exceed the value of one month's rent. Assistance is not available to households who have received assistance from the program within a 3-year period. Payments are made directly to the landlord. Homeless Prevention Programs - Prevention Services: Temporary financial assistance and/or supportive services to help people remain in their current housing situation, relocate, or divert them from entering the homeless system. Financial assistance may help with rental application fees, the first month's rent, past due rent, or utility assistance. - Rapid-Rehousing: Assists households to quickly exit homelessness and return to permanent housing. Provides one-time rental assistance and supportive services such as case management and housing navigation. Homeownership Down Payment Assistance - Assistance under this program may include payment of reasonable loan down payments, closing, and/or costs generally referred to as settlement charges associated with the first mortgage of the home to be used as the family's primary residence. Maximum amount of assistance is $3,000 including down payment, closing, and settlement costs. - Helps secure affordable homeownership for Fond Du Lac band members

Case managers are on-site to assist with accessing services such as housing, jobs, and education. - Minnesota Assistance Council for Veterans assists with benefits counseling - Housing referrals - Employment services

Works with individuals sleeping outside or in substandard places and assists them to get into permanent housing. Provides a non-emergency response to homeless concerns in public settings with a mobile outreach team able to visit individuals in the community. Can help address immediate basic needs for safety and survival outside with items such as food, personal grooming supplies, outdoor gear, clothing, and shelter referrals. Hennepin County residents who are experiencing homelessness can receive housing case management assistance through the Homeless to Housing program. The primary focus of the Homeless to Housing team is to fill system gaps by offering case management to people who otherwise lack support and access to resources by helping them secure permanent housing. The Homeless to Housing teams can help single adults, families, youth, and veterans who are staying in shelter or those living unsheltered with the following: 1. Referral 2. Intake 3. Emergency Shelter and Day Center for people experiencing homelessness. Servants of Shelter operates a 5 apartment unit Family Shelter. Shelter guests receive Case Management and Housing Resource Navigation services during their 30-90 day stay. SOS operates a Day Center with laundry, showers, a communal kitchen, hygiene items, and more. SOS also operates two Permanent Supportive Housing programs for people living with disabilities who are also experiencing homelessness.

A psychosocial rehabilitation program for veterans with a serious mental illness. Veterans work with a coach and as a group to create a recovery plan. Veterans work toward their goals by developing a recovery plan, attending programming 2 - 5 days per week, meet with a coach, explore educational services, and practice skills outside in the community.

Temporary emergency lodging for individuals and families who are homeless including: - Dinner - Sleeping accommodations - Light breakfast - Lunch - Showers - Guests must be assessed by the GRACE House Director or supervisor prior to acceptance at GRACE House - Guests will not be accepted after 8 pm - Overnight accommodations may be needed until assessment can be done - Day services available for registered guests - To volunteer please contact Grace House at (218)326-2790. Helps service members, veterans, survivors, and their families navigate the Veterans Administration benefits system. Members are assigned a Veteran Service Representative based on their location. A Veteran Service Representative may assist with the following: - Access benefits to disability compensation, pensions, survivors' pension, and other benefits. Must be temporarily disabled and unable to work for at least 30 days to be eligible for disability compensation. - Enroll in Veterans Health Administration (VHA) health care for medical, dental, emergency hospital care, nursing home benefits, and long-term care - Find mental health resources, military sexual trauma resources, mobile crisis teams, emergency suicide care, and treatment - Obtain military treatment and medical health records - Obtain burial benefits for veterans and eligible family members in federal, state, or private cemeteries - Request military discharge papers - Resolve and find resources such as housing assistance, homelessness, chemical health, and co-occurring disorders
