Home and Community Based Services Program - Advocate Homecare Community Service
- 24-hour emergency assistance: On-call counseling and problem-solving and/or immediate response for assistance at a person's home due to a health/personal emergency
- Community residential services: Services that provide training and/or habilitation, ongoing residential care and supportive services to adults and/or children in a setting licensed by the lead agency. These services are individualized and based on the needs of the person, as identified in the support plan
- Companion services: Non-medical care, supervision and socialization to a person age 18 or older
- Crisis respite: Short-term care and intervention strategies provided to a person due to the need for caregiver relief, protection of the person or others living with the person, or the person's need for behavioral or medical intervention
- Day support services: Individualized home, and community-based services (HCBS) that provide opportunities for community-based training and support for people with disabilities. Day support services develop and maintain essential and personally enriching life skills to ensure that people with disabilities can fully access and participate in personally preferred activities within their community
- Day training and habilitation: Services that develop and maintain life skills for people with developmental disabilities or related conditions so they can fully participate in community life
- Family residential services: Services that provide training and/or habilitation, ongoing residential care and supportive services to adults and/or children in a setting licensed by the lead agency where the license holder resides in the home. These services are individualized and based on the needs of the person, as identified in the support plan
- Homemaker services: Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living
- Individual community living supports (ILCS): For people who need reminders, cues intermittent/moderate supervision or physical assistance to remain in their own homes
- Individualized home supports without training: Services for adults or children when they need support, assistance and supervision in at least one of the community living service categories when living in their own home or family home. Community living service categories include community participation; health, safety and wellness; household management; adaptive skills
- Night supervision: Provides overnight assistance and monitoring by staff in the person's own home
- Respite care services: Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.

Errands and shopping assistance
- Volunteers will run local errands like picking up prescriptions or getting groceries for homebound seniors ensuring that they have the needed supplies to maintain their health and continue functioning independently at home
Friendly visits
- Volunteers provide social interaction for those who live alone or who would benefit from additional support and companionship
Handyworker and yardwork assistance
- Volunteers provide assistance with minor home repairs and maintenance (recipients are expected to pay for needed materials), outdoor chores such as garden projects, yard work, cleaning out gutters or snow removal
Housekeeping and chore service
- Volunteers provide assistance with routine cleaning and/or other occasional deep cleaning around the home to help ensure that seniors have a clean and safe environment which helps reduce safety risks that would be present if the senior attempted to do these chores on their own
Resource and referral
- Provides resources and referrals for any number of services that are not provided directly by agency
Respite care
- Volunteers provide the primary caregiver a temporary reprieve from their caregiving responsibilities and provide companionship and support to the senior receiving care. Faith In Action volunteers are not able to help with personal care such as toileting/bathing or assist with transfers or provide medical services such as administering medication or changing dressings.
Transportation
- Volunteer Drivers transport and escort participants to medical and non medical appointments helping increase mobility for seniors who are unable to drive or providing a safe alternative for those who still have a vehicle and license. Mileage reimbursement is available to all volunteer drivers.

Alzheimer's and Dementia Care: Provides individualized assessments for evaluating cognitive and functional levels. Services include:

Active Seniors: Provides adults 55+ with a variety of diverse activities such as exercise, wellness, and social/recreation designed to support independence
Care Consultation: Keystone Care Consultants help seniors navigate their health, economic, and social needs. Care managers meet with individuals in their homes or at one of the community centers to conduct an assessment, gather input from caregivers, and create goals. Once the assessment is complete, services may include: setting up rides to a social program, researching housing options, making a referral to an agency that helps with grocery shopping and/or cleaning, providing caregiver support, and accompanying seniors to doctor visits.
Grocery Delivery: Provides free grocery delivery for low-income seniors and adults with disabilities. Those who qualify can receive monthly deliveries. Delivery can include meat, dairy, fresh produce, and shelf-stable items. In each delivery, individuals will receive an average of 25 pounds of food per person in the household. Accommodations are based on customer choice models such as vegetarian, halal, etc. depending on what is available. Deliveries will be dropped off between 12:15 pm and 1:30 pm Tuesday - Thursday by a volunteer.
