In Home Care Service is a licensed service that provides an individualized assistance plan to help maintain and support healthy independent aging in the home and community. Aging individuals receive assistance in their home from a trained staff with a daily personal plan which can include household maintenance (laundry/cleaning), personal cares, and/or home management (phone calls, organizing, reading mail, arranging appointments), and socialization.

Provides advocacy and referrals to local services- 55 Alive driver's education- Chore program for residents of Clarkfield and Granite Falls, including yard work, one-time housekeeping, and other household tasks.- Exercise- Field trips/excursions- Tax assistance is available during tax season

Errands and shopping assistance- Volunteers will run local errands like picking up prescriptions or getting groceries for homebound seniors ensuring that they have the needed supplies to maintain their health and continue functioning independently at home Friendly visits- Volunteers provide social interaction for those who live alone or who would benefit from additional support and companionship Handyworker and yardwork assistance- Volunteers provide assistance with minor home repairs and maintenance (recipients are expected to pay for needed materials), outdoor chores such as garden projects, yard work, cleaning out gutters or snow removal Housekeeping and chore service- Volunteers provide assistance with routine cleaning and/or other occasional deep cleaning around the home to help ensure that seniors have a clean and safe environment which helps reduce safety risks that would be present if the senior attempted to do these chores on their own Resource and referral- Provides resources and referrals for any number of services that are not provided directly by agency Respite care- Volunteers provide the primary caregiver a temporary reprieve from their caregiving responsibilities and provide companionship and support to the senior receiving care. Faith In Action volunteers are not able to help with personal care such as toileting/bathing or assist with transfers or provide medical services such as administering medication or changing dressings. Transportation- Volunteer Drivers transport and escort participants to medical and non medical appointments helping increase mobility for seniors who are unable to drive or providing a safe alternative for those who still have a vehicle and license. Mileage reimbursement is available to all volunteer drivers.

Provides home health and personal care services that enable older adults to continue to live independently. Home care services are provided in the community or in a Sholom-owned facility. Provides a wide variety of free and low-cost home health care services. Services provided:- Blood pressure checks from an outreach nurse- Community education seminars designed for caregivers, seniors, family members, and the public on a variety of topics for older adults- Computer/technology help- Friendly visits from volunteers to help with transportation, chore support, grocery shopping assistance, meal preparation, plant care, pet care, telephone reassurance, or offer emotional support. Services are based upon volunteers and their availability to offer services.- Home safety checks to meet with older adults and discuss their needs, develop a care plan, and refer to professional services- Home chore assistance including seasonal yard work and occasional snow shoveling. Yard work may include spring and fall cleanup, installation of window AC's, and help around the house.- In-home foot care for non-diabetic individuals for a fee- In-home health assessment for a free in-home nurse visit to discuss health concerns and goals- Rides to medical appointments with at least 1-2 weeks' notice before an appointment

Non-professional services provided:- Errand running and shopping assistance- Friendly visiting- Light housekeeping- Meal preparation- Mail management and personal correspondence- Repairs and minor maintenance of home and yard, including snow removal. There is insurance coverage for volunteers during the times they are providing CREST services.??- Telephone reassurance- Transportation is available for appointments, church, social engagements, and shopping. Request for transportation must be done 2 days in advance and are not wheelchair accessible. Both curb-to-curb and door-to-door service is available depending on need. No fees and donations are accepted.- Volunteers do not give medications, do not assist in transferring sitting to bed, etc.

The Center advocates for state, local, and federal legislation.- Aging in Place Program: Offers accessible modification assistance, ramp installation, and accessibility assessments for homes and businesses for safe access and increased independence.- Assistive Technology Network: Assists individuals in the process of obtaining assistive technology devices.- Homemaking Services: Assists individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping.- Independent living skills training- Information and Referral: Provides up-to-date disability information and referrals to various agencies and individuals within the region.- On-Demand Lending Library: Program will purchase the requested device and the individual can use it for as long as needed; the program retains ownership of the item. Eligibility requirements: first come, first served basis; funds are limited.- Peer Mentoring: Assists individuals living with a disability to identify goals and create a unique plan to achieve those goals. Matches interested consumers with a peer mentor who provides support, problem-solving solutions, social skills, etc.- Public Education: Provides information, resources, and materials on accessibility guides, ADA, disability laws, etc.- Respite Care Services: Provides short-term in-home care and support when the primary caregiver or family members need to be absent from the home.- Systems Advocacy: Works to implement changes where needed at a broader systemic level.- Transition/Relocation Services: Assists individuals living with a disability in transitioning from nursing facilities and other institutions into community settings, with the goal of preventing institutionalization.
