Open to all in need of home health care in Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, Washington, and Wright counties; and the cities of Faribault and Owatonna.

Application process
Call, walk in, write, email or visit website for information and to request specific services. If requested, agency will schedule an assessment by a nurse within a week to determine services required. Services will be available about a week after the assessment.
Fees
Hourly fee varies with service provided. A sliding fee may be available on a case-by-case basis, call for information.

Provides the following services:- 24 hours/7 days a week availability- Errand running/shopping assistance- Grocery shopping- Homemaking assistance including meal preparation- Home health aides- Medication assistance- Personal care attendant- Physical, occupational, speech, and respiratory therapies- Private duty nursing (extended hour nursing including complex cases)- Skilled nursing care including private duty nursing
