Provides professional services, support, and individualized health care in the home setting. Services available include:Home Health Aide:- Daily Living Activity Assistance: Ambulation, bathing, dressing, eating, grooming, toileting, and transferring- Exercises and range of motionHomemaker:- Dusting- Laundry- Light housework- Meal preparation- ShoppingSkilled Care:- Medical social services- Nursing Services: Comprehensive terminal care, infusion therapy, medication supervision, newborn bili-blanket therapy, teaching, vein puncture for lab tests, and wound care- Rehabilitative Therapy Services: Occupational, physical, and speechHospice Program:- Hospice care can be given in the patient's home, a hospital, nursing home, or private hospice facility.- Nurse on-call 24 hours/7 days a week.- Enables patients to continue an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality.
