Services provided:- Blood pressure checks from an outreach nurse- Community education seminars designed for caregivers, seniors, family members, and the public on a variety of topics for older adults- Computer/technology help- Friendly visits from volunteers to help with transportation, chore support, grocery shopping assistance, meal preparation, plant care, pet care, telephone reassurance, or offer emotional support. Services are based upon volunteers and their availability to offer services.- Home safety checks to meet with older adults and discuss their needs, develop a care plan, and refer to professional services- Home chore assistance including seasonal yard work and occasional snow shoveling. Yard work may include spring and fall cleanup, installation of window AC's, and help around the house.- In-home foot care for non-diabetic individuals for a fee- In-home health assessment for a free in-home nurse visit to discuss health concerns and goals- Rides to medical appointments with at least 1-2 weeks' notice before an appointment
