Services provided: - Blood pressure checks from an outreach nurse - Community education seminars designed for caregivers, seniors, family members, and the public on a variety of topics for older adults - Computer/technology help - Friendly visits from volunteers to help with transportation, chore support, grocery shopping assistance, meal preparation, plant care, pet care, telephone reassurance, or offer emotional support. Services are based upon volunteers and their availability to offer services. - Home safety checks to meet with older adults and discuss their needs, develop a care plan, and refer to professional services - Home chore assistance including seasonal yard work and occasional snow shoveling. Telecare provides telephone reassurance to people living alone (i.e. homebound and elderly) who might require medical or emergency care. Volunteers provide daily phone contact to people who live alone and who feel reassured by outside contact. Clients call into Telecare between 8 am and 10 am every day. If a call is not received, a check-up call is placed promptly and if there is no response a relative or neighbor is contacted or, if necessary, the police are notified.
