Description

- Door-to-door transportation - Respite care for primary caregivers - Telephone reassurance - Friendly visiting - Meal preparation - Assistance typically one - two hours, once or twice weekly - Careteam formation and education within town or congregations - Shopping - Escorts to medical appointments - Transition home from hospital or nusing home - Light cleaning and chores - Educational workshops and individual consultations to help clients identify and find solutions to obstacles which make staying independent difficult
