Treatment and Recovery Services - Avivo

Chemical Health Assessments provided to people who meet eligibility guidelines, and Comprehensive Assessments once people are enrolled in Avivo's treatment program. - Family outpatient treatment (with housing option) for pregnant and/or parenting women with dependent children ages 0 - 17. (Rule 31 treatment programs, chemical health diagnosis, and/or co-occurring chemical and mental health diagnosis). - Individual, gender-specific outpatient treatment with housing option (Rule 31 treatment programs, chemical health diagnosis, and/or a co-occurring chemical and mental health diagnosis). - People in Avivo's treatment and recovery programs are eligible for enrollment in career education programs and employment services, as well as housing support. - Intensive Outpatient 245G Treatment Program for people who have a chemical health diagnosis and/or who have co-occurring chemical and mental health diagnoses. - Access to Avivo's other wraparound services, such as career education, Mental Health Clinic, housing supports, and family and employment services.

Transitional Residential Substance Use Disorder Services - Sunrise Care Facility - Spring Grove

Sunrise Care Facilities are designated by the State of Minnesota as Rate 2 Housing with Special Services. The men's and women's facilities have a capacity of 10 residents each. Residents work with staff on day-to-day tasks, utilizing copying skills and developing goals to eventually live on their own. Staff assist residents with completing medical, insurance, funding, and legal documents. Residents are encouraged to participate in community affairs and to seek employment when appropriate. Support for residents to attend AA meetings and seek sponsorship is provided including with transportation to meetings. Weekend passes and family visits are encouraged. Smoking is permitted outside. No mood-altering chemicals are allowed on the premises.

Non-clinical 24-52 week peer community-based rehab program: - Assists with transition to community support groups - Facilitates education and meetings on an outpatient basis. Group meetings occur three times a week and one-on-one meetings with a counselor occur weekly. - 12-step recovery programs - Reconnection with family and friends Offers transportation through a volunteer network for those unable to drive within a 15 mile radius of the agency.

Domestic Violence Prevention and Intervention: An intervention and healing program for Native women, girls, and Two Spirit people who have experienced intimate partner violence, including domestic violence, dating violence, sexual assault, and/or commercial sexual exploitation. Project CHILD - Hennepin County Human Services - Social Services

Project CHILD is for pregnant people ages 16 and older or who have been reported to Child Protection for controlled substance use anytime during their pregnancy. The program can also support those already in active recovery to reduce the risk of relapse during pregnancy. Services offered: - Help with basic needs including transportation - Substance use assessments, treatment services, case management, and counseling for up to six months after delivery - Peer recovery support specialist services - people with a lived experience in substance use and recovery - Prenatal care referrals, health assessments, and individualized care planning with a nurse - Prenatal and postnatal support groups. Parent(s) learn about parenting and baby development while building community with other parents. Also, offer support for all caregivers. - Project CHILD collaborates with many county and community-based resources for services. NOTE: Pregnant women under the age of 16 should be reported to the Child Protection Screening Intake line.

Alcohol and Chemical Dependency Treatment - Northland Recovery Center - Grand Rapids

Detoxification: - Removes toxic or harmful chemicals from the body. - Individuals are assisted by specially trained medical and counseling staff. - Available 24 hours a day, 365 days per year. Inpatient Treatment: - Provides intensive therapy in all aspects of recovery for those who are unable to maintain abstinence during outpatient treatment. - Residential stay is followed by an individualized schedule of outpatient treatment during day or evening hours. - Length of treatment varies according to the length of the residential stay, but typically averages 4-6 weeks for combined residential and outpatient treatment. Outpatient Treatment: - Intensive, conveniently structured treatment program for those individuals who are able to abstain from use of alcohol and other drugs while involved in the treatment process. - Treatment may be arranged during the day, evening, or a combination according to need. Guardianship Options - Lutheran Social Service of Minnesota - Metro

LSS Guardianship Option services include: Guardianship or Conservatorship: Manages the health and care of incapacitated individuals in the least restrictive manner possible and advocates in their best interest. Guardians and Conservators are appointed by the court. If an individual has no family or friends to assume the responsibility, LSS my be appointed. LSS staff gather information from individuals, caregivers and medical personnel to make medical and care decisions. Personal Representative of the Estate: Following court appointment, LSS completes probate requirements and handles disposition of assets according to the individual's wishes. Power of Attorney: Through a legally executed power of attorney, LSS is authorized to assist individuals in managing income and assets. Services may include check writing, checkbook balancing, assistance with financial and medical forms, assistance with retrofitting, maintenance, preparation for sale, the sale or purchase of property. Pooled Trust: Protects the public benefits of people with disabilities of any age while maintaining resources that will be available for their special or supplemental needs. The creation of a pooled trust moves assets from being countable for purposes of Medicaid and Social Security eligibility to being non-countable assets. Care Coordinator: Partners in decision making with individuals who may be alone and who may need help with contracting of repairs or services in the home, scheduling of appointments and making payments to providers.
