Home Care Specialized nursing care is provided by public health nurses in the home to assist with medication management, wound care, dressing changes, post-operative care, and other services ordered by the client's physician. The public health nurse may also assign a licensed practical nurse or home health aide to assist in the care of the client. Home health aides/elder care aides provide personal care (hair washing, showers, nail care, etc.), visiting, light housekeeping, and other services for clients with medical or health problems. Aides may also monitor vital signs and provide basic health services. The public health staff is available to perform home safety checks for families with children or for elders. A limited number of safety items may be available following the home safety check. Instruction on how to use this equipment will be provided, although the nursing staff cannot install the equipment for families. Community Health Representatives (CHR) Transportation Provides transportation for individuals to medical, dental, and optical appointments only after all other resources are exhausted. Appointments should be confirmed with the CHR's schedule to assure that clients arrive on time to their appointments. No one under the age of 18 will be transported without a parent. All persons being transported are required to wear a seat belt at all times. Delivers medical prescriptions to homebound elders who have no other means of obtaining their medication.

The Chore Services program assists older adults with a variety of in-home and outdoor services. Services include: - Assisted transportation with rides to the grocery store, doctors' appointments, and other errands - Homemaking services such as laundry, grocery shopping, and small meal prepration - Housing cleaning for deep cleaning and one-time decluttering - Minor home repair services and special projects - Yard maintenance all year-long such as lawn mowing, fall leaf raking, winter snow removal, and spring clean-up activities

Helps pay heat and electricity costs. Payments for energy bills are sent directly to the household's energy company or fuel provider for propane, fuel oil, or wood. Financial assistance varies depending on household income and fuel costs. Additional grants for eligible households who need help with the following services may be available: - Getting reconnected to heat and power after disconnections - Paying for emergency fuel deliveries of propane, heating oil, or biofuel - Preventing disconnections Other services may include: - Advocacy with energy suppliers and human service providers - Help with past due water and waste water bills - Education on efficient and safe use of energy - Repair or replacement of homeowners' broken heating systems The Reach Out for Warmth program provides crisis assistance, it is an emergency program for households facing a no heat situation. ROFW program is funded solely through local donations.

Provides resources for older adults living alone as well as caregivers needing support. Services are designed to help maintain independence as long as feasible. Home-based services are provided according to the availability of volunteers and staff: - Homemaking: Indoor light housekeeping services include mild laundry services, floor care, kitchen/bathroom cleaning, and other indoor help. Light "fix it" services (i.e., changing light bulbs, smoke detector batteries, etc.), meal preparation, menu planning, picking up groceries and medications, help with organizing bills, etc. Supportive programs: - Caregiver Consulting: Helps caregivers manage and live their lives while caring for another person, develop individualized plans, and connect with resources in the community - Caregiver Support Groups: Professionally facilitated, confidential environment for caregivers to share concerns, feelings regarding their role as caregiver, and to learn from each other

Volunteers take grocery shopping orders on Tuesday morning and volunteers shop for and deliver orders on Wednesday mornings. Clients need to be home at delivery time. Clients need to be 60+ and have difficulty shopping for groceries. A cost-share contribution is suggested based on income. Payments for orders are made by the client directly to the store.

Empowers people living with developmental disabilities to live independently in their own homes or apartments. Creates personalized support plans to help those supported achieve their goals. By providing assistance with daily living skills, staff help individuals live independently and connect with their community. Coaches assist with: - Arranging or providing transportation - Choosing personal attendants and housemates - Coordinating and supporting community involvement, like recreation and socialization - Grocery shopping, meal planning, and food preparation - Guidance and support in goal-setting and self-advocacy - Light housekeeping or paperwork - Money management, budgeting, paying bills, and banking - Registered Nurse (RN) case management or oversight - Scheduling and attending medical appointments - Selecting and moving into a home

Grants are sent directly to client's fuel vendor and/or electric vendor - Mail out applications available - Energy Related Repair Program is available to EAP eligible homeowners that occupies the home and has a hazardous and life threatening situation, has no heat, or has a malfunctioning heating system - Individuals should call for applications(or an application is on the bicap website available to download). Applications will be processed in the order in which they were received. If household is in an emergency situation, can call the Energy Assistance Program staff to see if immediate assistance is available based on funding and the completion of the application.
