The vehicle is equipped with high-speed internet, flat-screen TV, applications for programs, and meeting space for coffee and conversation.- Social: Offers card parties, coffee and conversation time, caregiver respite, karaoke parties, video calls and chats with friends, and social media assistance

Also provides a 12-unit transitional housing facility in Coon Rapids that offers supportive services to young people who are at risk of or experiencing homelessness.

Help applying for or renewing MNsure health coverage programs such as MNCare, Medical Assistance (MA), and other programs with support from a certified MNsure navigator

Services provided by advocates include:- Assistance with paperwork (i.e. medical claims, benefit applications, patient assistance programs, etc.) and with money management.- Home visits to assess needs- Information and referrals for housing, employment, Medicare and supplemental insurances, other government benefits, and transportation servicesAll services are free and confidential.??

Provides older adults with the opportunity to receive a home-cooked noon meal planned by registered dietitians; diabetic options available. Also offers a variety of activities, classes, and events for older adults. Health Care Access services assist with the following:- Determine if people are eligible for health care and financial assistance programs.- Help people with past medical bills by working with community care programs, collection agencies, and medical providers to discuss reasonable payment plans.- Meet with individuals and families to apply for programs- Provide referrals to other agencies for concerns related to financial assistance, housing, food, employment, and disability issues as needed

Statewide charity care program for Medicare beneficiaries. Participating providers agree to accept Medicare as payment for enrollees and waive out-of-pocket charges. The program is designed to bridge the financial gap between what Medicare pays to healthcare providers and what enrollees are responsible for paying out-of-pocket. ?A card is issued to those who qualify.Note: Medicare Partners (MP) is not insurance and does not pay any bills.
