Provides pregnancy resources, including: - Center has videos, literature, cassette tapes, and speakers available upon request- Emergency baby food, formula, and diapers- Free pregnancy tests- Learn to Earn program: Participants attend classes, read books, and watch videos to earn points, which are then used to "purchase" items such as baby furniture and baby clothing.- Maternity and infant/child clothing, baby care books, and toys- Pro-life counseling services- Staffed by trained volunteers

Provides a short-term crisis hotline for children and families who are struggling with problems such as child abuse, drugs, parenting concerns, suicide, family relationships, homelessness, etc.- Line is staffed by professional counselors. Provides Crisis Stabilization Services for those living with serious and persistent mental illness and co-occurring disorders, and who are currently experiencing a crisis or emergency situation.Crisis Stabilization Services help:- Coordinate with existing service providers and secure new resources.- Create a safe and fulfilling discharge plan.- Create an individualized treatment and recovery plan based on your needs and goals.- Maintain safety and sobriety by offering 24/7 supervision and one-to-one support.- Meet immediate needs for food, clothing, and medical care.- Stabilize symptoms by coordinating medication management and psychoeducation.

Provides personalized guidance and information, as well as coordination of resources and other services tailored to support and meet physical, social and financial care needs while maximizing independence and quality of life for adults and adults with disabilities, and their families, and those who are caring for them.- Individualized assessment: Care managers perform a comprehensive, multi-dimensional assessment which evaluates memory and cognition, health, daily functioning, safety, environment, support systems, and future planning- Liaison: Provides a communication resource and liaison for family members who cannot be present to keep them informed and involved- On-going consultation: Care managers can meet with family members to review the individual's situation and provide consultation regarding services, housing, care options, and other concerns. - Personal care plan: Care managers develop a tailored care plan which includes recommendations for services necessary to remain living safely at home, options available for other living arrangements, financial and legal planning tools needed to address vulnerabilities, problem-solving strategies, and other services and supports needed to enhance safety and quality of life. - Service coordination and monitoring: Care managers interact with health care and other professionals to ensure high quality, cost-effective services and care are provided. Our care managers monitor these supports to ensure needs are met on an ongoing basis. As care needs change, our care managers reassess and make recommendations about how to adjust the care plan to make certain that the individual's needs continue to be met.

Child and Family Health Services support families with the following services: - Breastfeeding Classes and Resources: Breastfeeding support services are available in the home, at the Horizon Public Health Office Alexandria main site, and via telephone.- Child Passenger Safety: Specialists can answer questions regarding child safety seats and check your car seats- Family Home Visiting: Registered nurses provide a variety of home visiting programs, including prenatal, postpartum, and ongoing parenting support services. - Follow Along Program: For parents of children ages birth - 5 at no cost to learn if the child is talking, moving, playing, and developing on track for his/her age.

Lands are held in trust for the benefit of the community by ensuring that it will always remain affordable for eligible homebuyers as their primary residence.Income-eligible homebuyers have access to homes at a 20-25% reduced rate from market-rate homes.
