Provides emergency food supplies to families. Food supply provided depends on the size of the family. Availability of the food shelf is once every 30 days or up to 12 times in a calendar year. Also accepts donations and provides volunteer opportunities.

Provides exterior painting and home improvement assistance for seniors, individuals with disabilities, and veterans or their surviving spouses so they may continue to live independently. Assistance is provided at no cost and is and is completed by volunteers between June 1 and October 1. Work is limited to exterior painting and related repairs only. Exterior improvement projects may include exterior painting, caulking/weatherstripping, repair of damaged siding or trim, repair of accessibility ramps or railings, and yard renewal.- Some homes may require advance preparation. Provides employment and training services to workers laid off from their jobs due to no fault of their own. Services include:- Career planning and personal counseling for career decision making, developing job goals, and personal planning- Job-seeking skills workshops on preparing resumes and cover letters, learning and practicing interviewing skills, internet job search, and networking- Placement services, including leads developed by counselors and agencies- Support services in certain cases are available for expenses such as child care and transportation, to enable participants to achieve their employment or training goals- Training and retraining resources to update skills or acquire skills in a new field

Independent living communities are maintenance-free, apartment homes and communities. Residents participate in social events and outings, dining in private dining room. Additional services are available such as housekeeping or health care.

Provides a hybrid crisis and Intensive Residential Treatment Service (IRTS) program which integrates mental health, medical, and substance use care in a 24-hour, supervised setting. Provides a high level of treatment and assistance in a structured environment.The Chicago Residence is a 13-bed single occupancy inpatient residence for men and women. For individuals experiencing a mental illness. The program includes:- 24-hour on-site staff and assistance in a residential setting- Assessment of immediate needs and factors that lead to the crisis- Short-term crisis stabilization and individualized treatment to address immediate needs and restore the individual to a pre-crisis level of functioning

Minnesota's Medicaid program for people with low income. Most people who have MA receive health care through health plans. Members who do not receive health care through a health plan receive care on a fee-for-service basis, with providers billing the state directly for services they provide.MA pays for a variety of services like doctor visits, prescriptions, and hospital stays. Some services and prescriptions may require prior approval. MA pays for medical bills going back three months from the month the application is received.NOTE: People with disabilities who work, even if they're over the income or asset limits of MA, may still qualify for a program called Medical Assistance for Employed Persons with Disabilities - MA-EPD.

Offers a community-based mental health service designed to assist individuals in managing their mental health conditions and achieving greater independence in daily living. The program focuses on providing support in various areas to promote recovery and enhance quality of life. Services included:- Daily Living Skills: Support in developing coping mechanisms, budgeting, cooking, nutrition, and household management.- Employment Support: Guidance in job applications, job seeking, and maintaining employment.- Healthcare and Benefits: Help in accessing and maintaining healthcare services and other benefits.- Home Visits and Wellness Checks: Regular visits to assess and support clients in their living environments.- Housing Support: Assistance in securing and maintaining stable housing.- Relapse Prevention: Education and strategies to prevent relapse and promote recovery.- Social Support: Encouragement of social interactions through support groups and community activities.

Provides general information on child care assistance programs to help families pay for child care for children up to age 12 and for children with special needs up to age 14. Child care costs may be paid for families while parents work, look for work, or attend school.

The assessment helps determine public programs that might pay for an individual's long-term services and supports or help them learn about other options if they don't qualify for publicly funded programs. It is required to determine eligibility for:- Community First Services and Supports - CFSS (formerly known as Personal Care Assistance - PCA)Home and Community-Based Services Waivers based on age:- Individuals Over 65 Years Old: Alternative Care (AC); Elderly Waiver (EW)- Individuals Under 65 Years Old: Brain Injury Waiver (BI); Community Access for Disability Inclusion Waiver (CADI); Community Alternative Care Waiver (CAC)Intellectual and Developmental Disability (IDD) Services (also known as Rule 185 Case Management)Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.
