Promotes a Community Wellness Partnership throughout the county in partnership with Cottonwood and Jackson County. Initiatives and services include: - Community Health Workers who work with individuals and families to help them navigate health care and social services systems - Diabetes and hypertension prevention with healthcare systems and local organizations to raise awareness - Healthy eating and physical activity in schools and communities - Wellness within the workplace to promote creation of healthier worksites through policy, systems, and the environment. Areas of focus include nutrition, physical activity, tobacco cessation/exposure, and breastfeeding support. - Tobacco cessation programs

Cholesterol/Blood Pressure Screening Clinics - Non-fasting and provide immediate results of an individuals total cholesterol and HDL cholesterol - Community Health Professional staff will provide on-site cholesterol screenings, education and referrals for groups, workplaces, health fairs or more - If interested call (218)283-7070 to schedule a site and a date - Cholesterol Screening Clinics are held every other month (odd month) on the 4th Tuesday from 1 pm - 4 pm at the Koochiching County Health Office - A small donation is requested for each individual screening Footcare Clinics: - Koochiching County Health Department 2nd Thursday - every month 9:00 am - 3:00 pm - Koochiching County Senior Center 4th Thursday - every month 9:00 am - 3:00 pm Outlying footcare clinics: - Julia O'Bert PHN 283-7070 for times and locations
