Provides resources, community education, a helpline, and a website for the public to raise awareness about advance care planning, palliative care, and hospice:- Advocacy for the needs of the terminally ill- Education for professionals and the general public regarding hospice, palliative care, and advance care planning- Free brochure "Choosing Hospice: A Consumer's Guide"- Promotion of increased access to quality hospice and palliative care for all Minnesotans- Referrals to over 70 hospice programs, and palliative care programs in Minnesota and over 1800 programs nationally- Questions answered and written materials about hospice, and palliative care and advance care planning provided- Some written materials are available in other languages
