Community Wellness & Recovery Navigator Program provides resources, non-clinical assistance, advocacy, and recovery support to individuals who struggle or are in recovery from a substance use disorder.

Provides substance use prevention, intervention, education, and support services to those in need. Services provided: - Co-Occurring Substance Use and Mental Health Disorders Outpatient Treatment Program: Offers a thorough assessment, treatment, peer support, and educational classes to adults. The outpatient program is designed to help individuals understand their substance use, mental health, and the negative impact it has on their lives. - Substance Use Assessments: Offers a comprehensive substance use assessment to adults and adolescents age 13 and older. The assessment is conducted to evaluate the person's substance use history, physical and mental health, medications, past treatment experiences, daily life, and legal issues related to substance use. - Substance Use Counseling and Group Sessions: Offers individual and group sessions for outpatient treatment and aftercare programs. Clients meet with clinicians to develop treatment goals and monitor progress. The program utilizes culturally specific and evidence-based practices such as Motivational Interviewing, Cognitive Behavioral Therapy, and recovery management skills.

Provides regular telephone peer-to-peer support calls to check in on recovery. Offers support, encouragement, and information about resources to help individuals seek or maintain recovery. Allows individuals making the calls to share their experiences, enhancing their own recovery.

- Coordination Of Care: Helps ensure a seamless and integrated approach to addiction treatment, where medical professionals, counselors, and support staff work collaboratively to tailor recovery plans that address the unique needs of each individual. This holistic strategy optimizes the rehabilitation process, providing continuous support and guidance through every step of the detox journey. - Detoxification Program: A program that is designed to provide short-term care on a 24-hour basis to detoxify clients and facilitate access to substance use treatment as indicated by a comprehensive assessment. - Group and Individual Counseling Sessions: Patients will be provided with individual and group counseling sessions that will implement motivational interviewing techniques, substance use disorders, and health education. - Peer Recovery Support Services: Mentoring and education, advocacy, and nonclinical recovery support provided by a recovery peer. - Withdrawl Management Program: A medically monitored withdrawal management program that operates around the clock to provide expert care, stabilize intoxicated patients, manage withdrawal symptoms, and provide access to substance use disorder treatment, as indicated by a comprehensive assessment.

Mental Health Unit (CORE): Responds to mental health calls. An officer and a homeless advocate team up to work together to get individuals to the right resources. Substance Use Response Team (SURT): Provides post-overdose outreach. Program divert individuals with substance use disorders away from the criminal justice system and into services that are supportive of their recovery. This is a peer recovery-led and run outreach program that does not include police in follow-up attempts but is instead embedded within a law enforcement agency.

Project CHILD is for pregnant people ages 16 and older or who have been reported to Child Protection for controlled substance use anytime during their pregnancy. The program can also support those already in active recovery to reduce the risk of relapse during pregnancy. Services offered: - Help with basic needs including transportation - Substance use assessments, treatment services, case management, and counseling for up to six months after delivery - Peer recovery support specialist services - people with a lived experience in substance use and recovery - Prenatal care referrals, health assessments, and individualized care planning with a nurse - Prenatal and postnatal support groups. Parent(s) learn about parenting and baby development while building community with other parents. Also, offer support for all caregivers. - Project CHILD collaborates with many county and community-based resources for services. NOTE: Pregnant women under the age of 16 should be reported to the Child Protection Screening Intake line.

Recovery plans and other supports are customized and built on each individual's strengths, needs, and recovery goals. Also educates individuals in treatment centers, detox, jails, public safety officials, and other community partners on substance use disorder and paths for recovery.
