The Itaskin Center & Cottage 35-Day Assessment Program is licensed by the Minnesota Department of Human Services (DHS). This assessment program serves residents between the ages of 10-17 years old. Upon completion of the 35-Day Assessment Program, a Comprehensive Assessment is provided to the youth's guardian and/or treatment team. The Comprehensive Assessment aims to identify child and familial strengths, resiliencies, and opportunities for growth via a multifaceted approach, while also providing diagnoses, recommendations, and a prognosis for recovery. This multifaceted approach involves a behavioral assessment, comprehensive assessment, wellness/recreation assessment, academic assessment, health screening/physical examination. Additional assessments, as requested by individual teams, include a psychiatric assessment and/or chemical use assessment. Upon completion of the program, the assessment team will also recommend a level of care believed to most accurately represent the current needs of the youth and how best to be supported. Opportunities for individual therapy and improving coping skills are provided throughout the 35-day assessment period.

Respite care services are provided for a few hours or a few days in one of the adult day centers. Overnight stays are available in one of the residential care communities, with a minimum stay of two weeks. Care is also provided in the individual's home.

Provides a break for caregivers who may be experiencing physical, emotional, or mental exhaustion from their caregiving responsibilities. It allows them to take some time off to rest and recharge while their loved one receives care from qualified professionals. This type of care can be provided for a few days, or even for an extended period, depending on the needs of the caregiver and the older adult being cared for.

Provided in the patient's home, a nursing home, or at an inpatient facility such as a hospital:- Care directed by a physician who has training in providing hospice and end-of-life care, along with the patient's personal physician- Family support services throughout the care- Help with daily needs, such as bathing, cooking, cleaning, or other needs- If requested, visits from a chaplain- Medical Equipment: Hospital bed, medical supplies, oxygen, and rails- Medications to help control pain and symptoms- Regular visits by nurses trained in hospice care, as well as 24-hour, on-call nursing support- Respite care- Social service support- Volunteer support

Goal is to enable adults to continue to enjoy quality lives, maintain independence, and provde a respite for families and caregivers. Provides activities and program that give participants the opportunity to develop confidence, self-esteem, self-expression, and to build on and maintain current living skills. Participants socialize with their peers through a variety of interactive activities and receive assistance with daily needs including: Bathing and personal care foot care Blood pressure checks Blood sugar checks Exercise program Health monitoring services Nail care Weight monitoring health assessmentsSocial, recreational and therapeutic activities are provided based on the individual's interest and level of functioning including: Baking and cooking Crafts Movies Outings Table games TriviaTransportation options available. Offers pick up and drop off service in the Cook area. Other options include community based services, local volunteers and family members.Services available on a full-time or part-time basis.Volunteers are needed for driving, assistance on outings, etc.

Groups of 3-5 older adults receive socialization, conversation, game play, outings, etc. from trained volunteers and staff (Group Facilitators), while caregivers receive 2-3 hours of respite. Group participants meet at a set location that matches their interests, and meet with individuals who are compatible with their personalities. The Groups are individualized for the participants.Transportation can be provided/included in the service.
