Offers person-centered support services to individuals living with a disability or mental health condition. Services provided:- Case management to assess gaps in services to develop a goal plan to resolve the individual's housing crisis and prevent them from becoming homeless- Develop life skills including self-sufficiency planning, establish independence, and housing skills- Help individuals secure and maintain stable housing. Services include house searching, application assistance, budgeting, landlord communication, and ongoing stability planning.- Meeting with staff for information about housing resources for rapid re-housing, transitional housing, permanent supportive housing, and eligibility. Provides referrals and resources to programs as needed.

Courses offered include English for Industry, Nursing Assistant, Basic Life Support for Healthcare Providers, and Nursing Assistant Refresher.- Re-Entry Services: Provides adults recently paroled or released an opportunity to gain proper credentials in trainings such as Class A Commercial Driver's License. In addition to job training and career planning, participants will have access to transportation assistance and financial coaching.- Welding and CNC Machining: Offers participants that have completed Manufacturing Foundations, the opportunity to pursue advanced manufacturing programs at Hennepin Technical College that includes 7 - 10 month certificates in Gas Metal Arc Welding or CNC Machining. Program also prepares participants an opportunity to continue their education and earn an associate's degree in a skilled trade.
