It is required to determine eligibility for: Community First Services and Supports - CFSS (formerly known as Personal Care Assistance - PCA)Home and Community-Based Services Waivers based on age: - Individuals Over 65 Years Old: Alternative Care (AC); Elderly Waiver (EW)- Individuals Under 65 Years Old: Brain Injury Waiver (BI); Community Access for Disability Inclusion Waiver (CADI); Community Alternative Care Waiver (CAC)Intellectual and Developmental Disability (IDD) Services (also known as Rule 185 Case Management)Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.

Children's mental health mobile crisis intervention and crisis stabilization services are intensive face-to-face, short-term mental health services initiated during a crisis to help the child return to the child's baseline level of functioning. This service is provided on-site by a mobile crisis response team outside of urgent care, inpatient or outpatient hospital settings.

Provides intensive, year-round academic services and enrichment activities to prepare low-income at-risk students holistically for success in college. Students attend Edison Senior High School, North Senior Academy, Patrick Henry High School, and South High School in Minneapolis. Students spend five weeks during the summer living at the university dormitories, attending classes, and related activities.Program includes:- Academic year courses in science, English, ACT, career prep, and college planning- Academic and personal counseling- Cultural and experiential activities?- Tutorial assistance in high school subjects

Caregiver Support Group is for caregivers of an ailing parent, child, spouse, or other individuals of any age. You are likely to face variety of new responsibilities, many of which are unfamiliar or intimidating. At times, you may feel overwhelmed and alone, and this support group will discuss issues or concerns you may be having.
