Identifies current services, develops and evaluates referral systems, and implements interagency Individual Family Service Plans

The assessment helps determine public programs that might pay for an individual's long-term services and supports or help them learn about other options if they don't qualify for publicly funded programs. It is required to determine eligibility for:Community First Services and Supports - CFSS (formerly known as Personal Care Assistance - PCA)Home and Community-Based Services Waivers based on age:- Individuals Over 65 Years Old: Alternative Care (AC); Elderly Waiver (EW)- Individuals Under 65 Years Old: Brain Injury Waiver (BI); Community Access for Disability Inclusion Waiver (CADI); Community Alternative Care Waiver (CAC)Intellectual and Developmental Disability (IDD) Services (also known as Rule 185 Case Management)Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.

A variety of opportunities to enrich the youth of the area including:- Service learning- Independent youth services- TREC (Tutoring, Reading, Empowering, Children)

Conducts community health education, including:- Karen chemical dependency program- Karen family planning program such as reproductive health, child spacing, and related topics- Mental Health and family relationships

MA-QWD is for employed people under age 65 with disabilities who lost their RSDI benefits because their income exceeds Substantial Gainful Activity (SGA) limits. QWD Program pays for Medicare Part A premium if eligible. This is the only benefit provided by QWD.

Services provided:- Investigates and treats Tuberculosis (TB)- Provides control function of managing communicable disease outbreaks- Schedules immunization clinics for child/adolescent/adult immunizations.- Screens and supports refugees and immigrantsVaccines provided under the Minnesota Vaccines for Children Program and the Uninsured /Underinsured Adult Vaccine program:- Adults: Tetanus, pneumonia, HPV (Gardasil), MMR (measles, mumps, rubella), Hepatitis A, Hepatitis B, Influenza, chickenpox, and shingles vaccinations. Any adult aged 19 or older who is underinsured or uninsured can receive these immunizations.?- Children: All required vaccines for day care, Headstart, and school (preschool - high school). Plus recommended vaccines including Hepatitis A, Human Papillomavirus (HPV), Rotavirus, and Influenza.

Provides housing with supportive services in a community setting. The building includes 30 units and is secured. Utilities are included in rent. Residents receive a noon meal and weekly housekeeping. Emergency pull cords are installed in each unit and are connected to Renville County Hospital next door. Medication dispensing and showers are available on-site.

The General Assistance (GA) Program provides single adults and childless couples with monthly cash grants for vulnerable persons whose income and resources are less than program limits. GA participants are also eligible for help with medical costs through the General Assistance Medical Care (GAMC) or the Medical Assistance (MA) programs and Food Support.

Pays for room and board for seniors and adults with disabilities who have low incomes. Aims to reduce and prevent people from living in institutions or becoming homeless.All recipients receive assistance each month to help pay for rent and food. Some recipients also receive Housing Support supplemental funding to provide other services if they don't qualify for home and community-based waiver programs, including but not limited to:- Arranging for medical and social services- Arranging for meetings and appointments- Assistance with transportation- Medication remindersThe individual's housing provider receives payments to pay for rent, utilities, food, household supplies, and other necessities.
