Supported living settings are homes for up to four people with physical, medical, mental health and behavioral support needs. They combine a positive, nurturing environment with personalized supports and 24-hour supervision.Through customized care plans, individuals receiving support work toward achieving their goals, such as living independently in an apartment or cooking healthy meals. Services promote development in the areas of social skills, recreation, meal preparation, budgeting, self-care, and educational or vocational interests.Individuals form relationships with skilled and caring direct care professionals and nursing staff who ensure their everyday needs, as well as overall medical requirements, are being met.

Provides establishment and enforcement of child support orders. Nurses can provide medical advice on what potential options are available such as virtual care, primary care, urgent care options, or seek emergency care if needed at a Hennepin Healthcare location

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers.Works to strengthen families and engage the ability of all parents to provide an environment that supports the healthy development of their child. Services may include:- Book and toy lending library- Dad and child programs- Early childhood screening for children ages 3 - 5- Home visits- Information on community resources- Parent-child activities- Parent discussion groups- Play and learn activities for children- Special events for the entire family- Programs for non-English language learners- School readiness program for children ages 3 1/2 - 4- Single parent program- Workshops on specific topics

Minnesota's Medicaid program for people with low income. Most people who have MA receive health care through health plans. Members who do not receive health care through a health plan receive care on a fee-for-service basis, with providers billing the state directly for services they provide.MA pays for a variety of services like doctor visits, prescriptions, and hospital stays. Some services and prescriptions may require prior approval. MA pays for medical bills going back three months from the month the application is received.NOTE: People with disabilities who work, even if they're over the income or asset limits of MA, may still qualify for a program called Medical Assistance for Employed Persons with Disabilities - MA-EPD.

If the cost of the item exceeds $600, it is the applicant's responsibility to pay the remainder of the balance and they must demonstrate the ability to pay the remaining balance.Requests are generally limited to 1 request per household every 2 years, unless the request is for an emergency beyond the household's control.

Memory care facilities offer a safe and familiar environment where residents with dementia or other memory concerns receive dedicated care from trained caregivers. Each resident occupies a private apartment and receives a personalized program of care tailored to them. Memory care residents experience specialized care and security.

The amount of assistance provided might not cover the entire emergency, but it can help.Emergency General Assistance provides short-term, one-time assistance for persons in a financial crisis because of fire, flood, storm, illness, accident, theft, utility shut-off, eviction, foreclosure, moving expense, or other emergency need.

Provides a supplemental food program that helps families eat well and stay healthy by providing nutrition education, one-on-one counseling about nutrition, vouchers to buy healthy foods, support and help with breastfeeding, and referrals to health care, immunizations, and other programs.
