Offers one and two-bedroom subsidized apartments for older adults and adults with disabilities

Volunteers take grocery shopping orders on Tuesday morning and volunteers shop for and deliver orders on Wednesday mornings. Clients need to be home at delivery time. Clients need to be 60+ and have difficulty shopping for groceries. A cost-share contribution is suggested based on income. Payments for orders are made by the client directly to the store.

Provides a therapeutic, relationship centered preschool day treatment setting for young children with emotional or behavioral challenges. The program uses a trauma informed approach based on the Developmental Repair Model and focuses on supporting children through consistent routines, structured activities, and collaborative work with families. The day treatment program includes the following model:- Children participate in a small classroom with up to seven peers and three to four mental health staff. The daily schedule mirrors a typical preschool setting and includes planned activities, play, and social time that promote emotional, social, and relational skills. Some classrooms also offer embedded services such as speech therapy and occupational therapy.- Families take part in regular skills building sessions and therapy to support their child's progress across home and community settings. Staff can assist families in understanding insurance and funding options, and medical transportation may be available for those with state funded insurance.- Promotes healthy development by identifying emotional, social, and developmental needs, providing supported opportunities to practice new skills, and strengthening parent child relationships.

Minnesota's Medicaid program for people with low income. Most people who have MA receive health care through health plans. Members who do not receive health care through a health plan receive care on a fee-for-service basis, with providers billing the state directly for services they provide. MA pays for a variety of services like doctor visits, prescriptions, and hospital stays. Some services and prescriptions may require prior approval. MA pays for medical bills going back three months from the month the application is received. NOTE: People with disabilities who work, even if they're over the income or asset limits of MA, may still qualify for a program called Medical Assistance for Employed Persons with Disabilities - MA-EPD.

Direct Access allows an individual to go directly to a provider they choose to receive a comprehensive assessment and access care immediately. The county will assess individuals who need help paying for a chemical use assessment or treatment services through the Behavioral Health Fund. Services include:- Designated staff that provide chemical dependency assessments- Referral to local community service providers for chemical assessments or treatment services

Provides investigative and intervention services for child abuse, neglect, and/or abandonment. Services include:- Accepts reports of abuse- Assessment of the initial referral- Consultation with individuals who suspect abuse- Consultation with the family to determine the nature of the problem- Dependency investigation- Explanation of reporting statutes and answer questions regarding agency practices- Ongoing support services for children and their families- Temporary placement of children who are removed from the home- Works with the family toward unification or reunification
