Provides various health and wellness programs to the community, such as:- Diabetes Awareness Campaign: In partnership with the Minnesota Department of Health, CLUES provides education and information on prediabetes, type 1-2 diabetes prevention, and diabetes management to empower people to improve their health. - Es My Time Tobacco Cessation Support and Resources: Offers one-on-one resource navigation for tobacco users, tobacco cessation education groups, and supports Latino-serving agencies and networks- Mayores Majores Dementia Program: Helps the Latino community and their families learn more about Dementia through support groups, group activities, social connections, and education workshops- Sexual Health Education: Provides comprehensive sex and sexuality education workshops for parents and teenagers focused on values, consent, and communication. Offers parent workshops conducted in Spanish on how to talk about sex education, sexuality, and sexual health. Also, provides youth workshops in English and Spanish about sex education and sexuality tailored to the learning needs of the students.Services provided by Promotores de Salud (Community Health Workers).

Provides an outpatient treatment program for individuals who are chemically dependent and/or have co-occurring disorders. A licensed 245G substance use disorder treatment program. Clients will need to have a substance use evaluation to determine the level of treatment they need. Additional services offered:- Co-occurring disorders treatment program- Group counseling to gain perspective, connection, and healing - Intensive Outpatient Program (IOP): A treatment option for adults ages 18 and older who need more intensive services toward recovery while continuing to live at home, work, and be active in the community- Substance Use Assessment and Counseling: Provides a comprehensive substance use assessment to establish the type and severity of the substance use disorder diagnosis. Also, offers one-on-one counseling with a licensed alcohol and drug counselor for adolescents and adults.

Increases access to medical services by traveling from site to site to bring services where it is convenient for individuals facing homelessness. Services include:- Chemical health assessment and referrals- Chiropractic services- Medical services include primary care, transportation assistance, and community resource assistance and referral- Nutrition services include assessment and referral, education, consultation, advocacy- Mental health diagnosis and treatment services- Optical including vision screening and eyeglasses- Pharmaceutical services- Physical therapy- Public health nursing services include outreach and assessment, maternal/child health and family planning, and TB case managementDonations of unopened medicine, inhalers, and supplies are gratefully accepted. Must be delivered to the project.

Services include:- Care Coordinator: Partners in decision-making with individuals who may be alone and who may need help with contracting repairs or services in the home, scheduling appointments, and making payments to providers- Guardianship or Conservatorship: Manages the health and care of incapacitated individuals in the least restrictive manner possible and advocates in their best interest. Guardians and Conservators are appointed by the court. If an individual has no family or friends to assume the responsibility, LSS may be appointed. LSS staff gather information from individuals, caregivers, and medical personnel to make medical and care decisions.- Personal Representative of the Estate: Following court appointment, LSS completes probate requirements and handles the disposition of assets according to the individual's wishes- Pooled Trust: Protects the public benefits of people with disabilities of any age while maintaining resources that will be available for their special or supplemental needs. The creation of a pooled trust moves assets from being countable for purposes of Medicaid and Social Security eligibility to being non-countable assets.- Power of Attorney: Through a legally executed power of attorney, LSS is authorized to assist individuals in managing income and assets. Services may include check writing, checkbook balancing, assistance with financial and medical forms, assistance with retrofitting, maintenance, preparation for sale, the sale or purchase of property.

Specialized and Whole Family Foster Care:Offers emergency, short-term, long-term, and whole-family foster services. Provides an intensive treatment program in a foster care environment for youth with specialized treatment needs. Provides support for at-risk parents. Skilled foster parents work with families to teach parenting skills through modeling parenting techniques, setting examples and providing education.

Offers 39 residential apartments as part of its comprehensive support for single mothers and their children. These units are designed to provide safe and affordable housing, enabling mothers to pursue higher education and career goals without compromising their family's well-being.Also provides an on-site licensed child development center for resident children. Offers early childhood education, supporting the program's two-generation model aimed at disrupting the cycle of poverty for single mothers and their children.

Provides cash assistance to help adults receiving Supplemental Security Income - SSI pay for their basic needs. Adults who are blind, have a disability, or are age 65 and older but don't qualify for SSI because of their income may still be eligible for MSA if they meet income requirements.The basic monthly MSA grant is calculated using either the maximum SSI amount (Federal Benefit Rate) for the adult or their countable income (if they do not receive SSI), and the MSA standard set by state law.Special Needs GrantMSA also may provide special payments for adults who need:- Certain home repairs- Certain household furniture and appliances- Guardian or conservator service fees- Help paying for housing costs through MSA Housing Assistance- Medically prescribed special diets- Representative payee servicesAdults who receive MSA are also eligible for help with medical, food, and housing costs through Medical Assistance - MA, the Supplemental Nutrition Assistance Program - SNAP, and MSA Housing Assistance programs.

Cash-grant program for families with low incomes experiencing household emergencies. Families can receive assistance once every 12 months. The assistance received must resolve the crisis. Families must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help. The types of assistance the program may help with are for rent assistance, rent deposit, electric, gas, heating fuel, water, burial assistance, or other emergency need based on available funding.

Provide assistance and support to veterans and their dependents in obtaining benefits through county, state, and federal programs. Helps victims who have been impacted by domestic violence in Carver and Scott counties. Services provided:- 24-hour confidential crisis line to provide a safe place to call, receive support, and ask questions. Trained advocates are available 24 hours/7 days a week. Meetings at the office are also available by appointment.- Emotional support for the victim and their child(ren)- Help victims fleeing a domestic violence situation seek safe shelter- Information for support group, resources, school enrollment, and follow up services- Safety planning and safety equipment for the home- Partners with Carver Scott Humane Society to offer temporary fostering of pets for Southern Valley Alliance clients who are residents through their program, SafeFurSpaces, part of the Fur Keeps Program- Transitional Housing: Partners with Carver County Community Development Agency (CDA) and private property managers to connect clients to scattered site transitional housing units located throughout Carver and Scott counties. Housing is offered on a case-by-case basis, depending on funding and size availability of apartment units.

Provides work experience to individuals interested in a career in criminal justice. Positions are available to students seeking to fulfill the requirements of their academic programs as well as professionals with post-secondary education who are interested in obtaining unpaid work experience.

Services include:- Home health aides who can help with bathing, dressing, grooming, and other delegated tasks- Occupational therapy service to help clients recover from injuries, disabilities, or illnesses and relearn everyday activities- One-on-one personal support service to support individuals to become more independent- Personal care attendants and homemakers to assist with basic home skills such as eating, transfers, mobility, positioning, toileting, light housekeeping, and laundry- Physical therapy service to help clients improve their mobility, balance, prevent falls, and recover from previous health concerns- Skilled nursing services providing initial assessment, health education, medication setup, diabetic management, wound care, health management, and more
