Assertive Community Treatment (ACT) provides intensive services for people struggling with chronic disorders. ACT works to enhance wellbeing and reduce repetitive hospitalizations.

Provides care at the clinic for common ailments such as:- Adult and child immunizations; flu vaccine- Bladder and urinary-tract infections (UTI)- Check-ups and annual physicals- Chronic disease management (for diabetes, hypertension/high blood pressure, and other diseases)- Cold and flu symptoms- Ear infections- Headaches- Health, weight, and vision screenings- Insect bites- Joint and back pain- Nausea and vomiting- Obstetrics and Gynecology (OB-GYN)- Pediatric care and back-to-school physicals- Preventive care- Rashes- Physical exams- Radiology and imaging- Reproductive health, including family planning, and counseling- Sinus infections- Stomach problems- Wellness and preventive care

Provides nutrition education and special foods which promote good health for pregnant, breastfeeding, and post-partum women with infants and/or children up to age 5.WIC provides the following services:- Assistance with any special needs- Health assessment and screening- Nutrition education- Referral to other community servicesVouchers are given to participants to purchase nutritious foods such as cereal, cheese, eggs, infant formula, fruits and vegetables, milk, and peanut butter.

The Forest History Center is located on a 171.5 acre site endowed with forests, grasslands, and wetlands that are abundant with wildlife. The center endeavors to tell the story of how people have and do use and manage the forests on northern Minnesota, while also serving as a unique environmental learning center. More then 5 miles of trails are available for hiking and biking

Provides culturally sensitive Adult Rehabilitative Mental Health Services (ARMHS) that can be provided in the office, client's homes and other appropriate community locations.Provides culturally sensitive outpatient therapy and counseling services as well as diagnostic evaluations and psychological assessments for adults and youth ages 13 - 19. Conditions treated include:- Anxiety- Attention-Deficit/Hyperactivity Disorder- Autism Spectrum Disorder- Depression - Family problems- Grief- Life transitions- Post-Traumatic Stress Disorder- Post-partum depression- Severe and persistent mental illness- Stress-related conditionsIndividual, couple, family, and group therapy sessions are available. Telehealth services are available.

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers.ECFE works to strengthen families and engage the ability of all parents to provide an environment that supports the healthy development of their child.Services may include:- Book lending library- Dad and child programs- Early childhood screening for children ages 3 - 5- Information on community resources- Outdoor classroom- Parent-child activities- Parent discussion groups- Play and learn activities for children- Programs for non-English language learners- School readiness program for children age 3 1/2 - 4- Special events for the entire family- Workshops on specific topicsNOTE: Call for current class offerings and schedule of events.

Head Start Head Start is a center-based preschool program for children 3-5 years old. Weekly home visits include:?- Assisting pregnant women with accessing comprehensive prenatal care to promote healthy development and a safe delivery.?- Information and referrals to other community agencies as needed.?- Parenting information and screenings to ensure that children are developmentally on track.?- Playgroup sessions are provided throughout the year where parents and children can build friendships and discuss interesting parenting topics.?- Supporting parent's efforts to meet the challenges of parenting.?- Working with parents to ensure that children have access to quality health care, including dental, vision, speech, hearing, nutritional, mental health screenings, and physical exams and immunizations.Pregnant women and children up to 3 years old may apply for this year-long opportunity.

Pays for room and board for seniors and adults with disabilities who have low incomes. Aims to reduce and prevent people from living in institutions or becoming homeless.All recipients receive assistance each month to help pay for rent and food. Some recipients also receive Housing Support supplemental funding to provide other services if they don't qualify for home and community-based waiver programs, including but not limited to:- Arranging for medical and social services- Arranging for meetings and appointments- Assistance with transportation- Medication remindersThe individual's housing provider receives payments to pay for rent, utilities, food, household supplies, and other necessities.

Allows an individual receiving PCA services to manage their own personal assistants; PCAs provide services to people who need help with activities of daily living, housekeeping, meal preparation, etc.

A drop-in center for individuals with psychiatric illnesses offering socialization opportunities, classes on a variety of topics, job searching/resume writing assistance, etc. A new calendar of events is available each month and can be found online. Staff work with clients to find a class or activity that is a good fit for the client.
