Provides investigation and intervention services on behalf of vulnerable adults who are unable to act on their own behalf, manage their own affairs, or are in immediate danger of abuse. Types of abuse may be physical, emotional, or sexual abuse, unsafe or hazardous living conditions, exploitation, neglect, or abandonment. Services include:- Taking reports and investigating abuse and/or maltreatment- Removal of the individual to safer surroundings- Services necessary to remove the conditions that have created a threat to life?NOTE: Complaints, questions, or concerns involving incidents in facilities licensed by the Health Department, such as hospitals, nursing homes, boarding care homes, supervised living facilities, assisted living, and home health agencies can be referred to the Minnesota Board on Aging Ombudsman for Long-Term Care.

Offers food shelves once a month by appointment to receive fresh produce, frozen meat, shelf-stable foods, bread and bakery items, personal care products, household cleaning supplies, and pet food when available. In addition, they may visit weekly for fresh produce, dairy, and bread/bakery items. Donations are accepted during food shelves' open hours and may include canned and boxed foods, cereal, peanut butter, pasta, rice, juice, pet food, dish and laundry soap, cleaning supplies, toilet paper, soap, deodorant, toothpaste, and shampoo/conditioner.

Services are open to individuals and families to help individuals manage their mental health symptoms, help with medications, housing, employment, and recovery.- Targeted Case Management: Coordinated care and community support for adults. Offers one-on-one support for adults living with a mental illness. Case managers connect adults with mental health care, medical services, housing, and community services.- Therapy Services: Personalized trauma-informed therapy for adults and families to help clients navigate life's challenges such as anxiety, depression, trauma, grief, parenting challenges, loss, conflict, and relationship issues. Services are provided in-person and telehealth.- Recovery Services: Strength-based care for individuals and families navigating substance use and recovery. Offers an intensive outpatient program providing same-day evaluations, relapse prevention, and peer services. Group therapy services are offered in relation to recovery, providing resources, education, and support.

Intake process for programs serving individuals and families who are in search of housing programs or are in need of financial assistance for housing.Family Homeless Prevention and Assistance Program: Helps prevent homelessness by providing one-time assistance with a rent or mortgage payment for households until their financial situation can stabilize. Offers case management and follow-up services.Rapid Rehousing Program: Provides information, referral, advocacy, and first month's rent and/or security deposit assistance to individuals experiencing homelessness, doubled up or homeless individuals moving into a housing unit. Ongoing rent subsidy may be provided until the household is able to sustain their rental housing.
