Single adults who have long histories of homelessness compounded by problems of mental illness/emotional issues and substance abuse (including alcohol, cross addiction, amphetamine, cocaine, opiate, prescription drug and smoking) who are residents of Anoka, Carver, Dakota, Hennepin, Ramsey, Scott and Washington counties

Payment Options
Provides permanent and short-term supportive housing services for people who have had difficulty maintaining housing, and those struggling with a diagnosed mental illness. Helps find stable, affordable housing in their community of choice and then provides supportive services so they can remain in their residence, engage in their community, and reach their broader goals. Using a housing first approach, the supportive housing services help people achieve increased levels of stability and satisfaction with their lives while encouraging the autonomy of independent living. The program goals are to help people break the cycle of homelessness and to improve their health and self-reliance. Flexible services are tailored to individual needs.
