Application process
Call to schedule an assessment by phone or in-home by an Registered Nurse (RN). The assessment can usually be scheduled for one to two days. An assessment through the county is necessary to receive a Personal Care Attendant.

Offers in-home health care services to tailor to client's needs and help them remain independent without staying at a facility or hospital. Services include: - Home health aides who can help with bathing, dressing, grooming, and other delegated tasks - Occupational therapy service to help clients recover from injuries, disabilities, or illnesses and relearn everyday activities - One-on-one personal support service to support individuals to become more independent - Personal care attendants and homemakers to assist with basic home skills such as eating, transfers, mobility, positioning, toileting, light housekeeping, and laundry - Physical therapy service to help clients improve their mobility, balance, prevent falls, and recover from previous health concerns - Skilled nursing services providing initial assessment, health education, medication setup, diabetic management, wound care, health management, and more
