Provides home health care services to meet the needs of the individual residing in their home. Home care plans are established to provide services such as: - Ambulatory and transfer services within the home for individuals who need assistance - Assistance with personal care for bathing, dressing, and grooming - Homemaking services to help with sweeping, mopping, dishes, cleaning, changing linens, and laundry assistance - Home safety assessments - In-home respite care - Medication management - Nursing evaluations and nursing care - Prepare meals - Referrals to appropriate community resources
