Eligibility

  • Be 18 years old or older- Be eligible for Medical Assistance (MA), MNCare, IM Care, UCare, etc;- Have been diagnosed with a mental illness that causes impairments in their daily activities, relationships, and/or employment- Residents of Hibbing and surrounding communities within 10-mile radius, International Falls, and Itasca County
