Provides the following services: - Accompaniment to medical appointments - Assessment of needs - Care coordination and consultation by licensed nurses and social workers - Caregiver coaching - Facilitation of family meetings and professional care conferences - Information and referral to services - Liaison for family members with health care providers - On call only services that provide 24/7 support and guidance in the event of a crisis - Review of current Medicare, Medicaid, and private insurance coverage, providing explanation of benefits and assistance with organizing and submitting health insurance claims and/or hospital bills
