Ensures safe environments and services for adults who may not be able to protect themselves Note: To make a vulnerable adult report, call the Minnesota Vulnerable Adult Reporting Center at (844)880-1574. Reporting center operates 24 hours/7 days a week. If someone requires immediate assistance call 911. Complaints, questions, or concerns involving incidents in facilities licensed by the Health Department; such as, hospitals, nursing homes, boarding care homes, supervised living facilities, assisted living and home health agencies can be referred to the MN Office of Health Facility Complaints at (651)215-8702 or toll-free at (800)369-7994
