Enrollment happens automatically when a clinic is participating. To find a participating clinic, call or check the website. Those who do not want to be part of the registry can tell the clinic or call the intake number.

A confidential, computerized information system, also known as an immunization registry, which contains a complete and accurate record of a child's immunizations, no matter where they got those shots. This ensures that Minnesotans get the right vaccines at the right time. Parents and their doctors can get their child's shot information even if the family has changed clinics often. This gives the parents a record needed for day care, school, sports, etc. It also helps prevent a duplication of shots from different clinics. The registry will send a reminder notice when shots are due.
