Financial Assistance - M Health Fairview Clinic - Cottage Grove

- Charity Care: Fairview patients who cannot pay all or part of their medical bills. Patients may qualify for discounted care based on their household income, family size and assets. Income may be up to nearly 3 times (300%) the Federal Poverty Level. - Pharmacy Assistance Fund: Eligible Fairview patients typically have no prescription drug benefits or have exhausted their coverage. They also must not be eligible for, or have access to, alternative sources of coverage or funding (such as Medicaid, MinnesotaCare or Medicare). Total gross annual household income is at or below 2 times (200%) the Federal Poverty Level. - Uninsured Discount: Fairview patients with a household income equal to or below $125,000
