Members of the Bois Forte Band of Chippewa who are residents of Itasca, Koochiching or Saint Louis counties

Required documents

Application letter stating need. If asking for assistance for childcare, a statement including the full name of the provider, address and childcare rate is required, along with a W-9 form from the provider.

Description

Helps with uniforms, work clothing, work boots/shoes, tools, gas, auto expenses (excluding routine maintenance, cosmetic repairs and/or loan payments) and childcare. Amount of financial assistance is based on verified need, not to exceed $400. Payments are issued in the form of a purchase order or direct payment to the provider of services.
