Services are designed to support individuals with significant functional impairment who need intensive, community-based care to maintain stability, prevent hospitalization, or support community reintegration.Services include individualized support with daily living needs, mental health symptom management, medication support, financial management, housing and employment assistance, and referral to substance use and other community resources.
