An assessment is required to access any substance use disorder services- The assessment will determine the individual's diagnosis and level of care recommended- Serves residents of Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, Washington, and Wright counties

Evaluates the impact of substance use on an individual and their family and determines appropriate treatment recommendations. Provides referrals to substance use treatment and to services addressing underlying or co-occurring issues. Assessments are offered in person and via telehealth.
