Men, women, youth, and children in Ramsey County who are homeless or in emergency shelters

Increases access to medical services by traveling from site to site to bring services where it is convenient for individuals facing homelessness. Services include:- Chemical health assessment and referrals- Chiropractic services- Medical services include primary care, transportation assistance, and community resource assistance and referral- Nutrition services include assessment and referral, education, consultation, advocacy- Mental health diagnosis and treatment services- Optical including vision screening and eyeglasses- Pharmaceutical services- Physical therapy- Public health nursing services include outreach and assessment, maternal/child health and family planning, and TB case managementDonations of unopened medicine, inhalers, and supplies are gratefully accepted. Must be delivered to the project.
