- Application/registration form, Social Security number, two identical passport photographs, proof of United States citizenship (certified copy of record of birth in the United States or original naturalization certificate. If adult applicant is not applying for the first time, the most recent previous United States passport is preferred proof of citizenship.) and current identification (driver's license with signature and photograph) - Parent's identification may be presented on behalf of a minor under age 18, when the minor does not possess his or her own identification. Parents must be present, with their identification, when applying for a passport for child under age 16.
