Individuals living with an intellectual disability, developmental disability, or mental health diagnosis as well as their families/caregivers who are residents of Anoka, Dakota, Hennepin, Isanti, Ramsey, Scott, Stearns, Washington, and Wright counties in Minnesota

Designed for people who choose to remain at home with families and loved ones. Provides structure, guidance, and ideas to help the individual while supporting health and safety needs. Also offers services and training to aid family members and caregivers. In-home support topics and training services include: - Academics - Behavior support and intervention - Community participation - Crisis and safety planning - Effective communication - Financial planning - Individual and family resilience - Parenting skills - Peer relationships - Risk and safety - Self-advocacy - Self-control - Skill, behavior, and risk assessments - Transition
