- Service of Grand Itasca Clinic and Hospital. - The program is designed to provide skilled nursing, therapy, and home infusion therapy to patients with severe illness or injuries. - Home Care services are directed by the physician. - Home care is reimbursed through Medicare, Medical Assistance, private insurance, VA and waivered services. - Self pay is also possible. - Rehabilitation Services (Physical, Occupational and Speech Therapy) - Skilled Nursing Services - IV Infusion Therapy - Case Management - Dietary Consultation - Respiratory Therapy Support - Pharmacy Support
