- Call or walk-in to a drop-in center or shelter to connect with a street outreach worker for a Coordinated Entry Assessment - A referral is required through the Hennepin County Coordinated Entry process. This process is open to individuals who are staying in an emergency shelter or working with a street outreach team and sleeping outdoors (camp, car, transit, etc). Individuals currently staying with friends or family, or who are in treatment, will not be able to access Coordinated Entry.

24-unit housing program that provides housing and case management services for youth who are experiencing homelessness. Supportive services offered include: - Assistance with education and employment - Connections to health care services - Life skills education - Leadership development opportunities - On-site mental health and chemical health services - Support groups focused on building community with peers
