Families with children ages birth - 3 years who are residents of Hennepin County

birth certificate for families with children
photo ID
proof of residency (piece of mail, application of new ID, rental lease, bus ticket, etc.)
shelter verification
verification of the last 30 days of income and assets
any supporting documents can help process applications quickly

Application process
- Call for more information and to enroll. Staff will mail out forms to be filled out with a return envelope.- Participation is voluntary. Developmental tracking for children ages birth to age 3 through a series of age appropriate questionnaires for parents or caregivers. Services include: - Periodic developmental tracking - Age-appropriate activity sheets for child - Parent education information - Information and referral in accessing needed servicesNOTE: Children receiving Early Childhood Special Education services do not qualify for the Follow Along Program as it would be a duplication of services
