Description

Public Health nurses provide services listed below. Health Services - Telehealth screening and assessment for COVID-19 symptoms - Blood pressure and blood glucose (diabetes) screening - Cholesterol screening - Over-the-counter prescription assistance - Pre-diabetes testing - Pregnancy test verification for medical assistance - Medical follow-up for diabetes, hypertension (high blood pressure), post-surgical procedures, Emergency Room visits, etc. - Health promotion education on multiple topics such as nutrition; healthy pregnancy; infant and child growth and development; smoking cessation; weight management; chronic disease management, etc. Community Resource Connections - Financial screening for program eligibility for prescription drug assistance; Women, Infant and Children (WIC); Head Start; Medical Assistance; Energy Assistance and food shelf - Medication disposal
