Native Americans who live on the Fond du Lac reservation within the towns of Cloquet, Brookston, Sawyer, and Duluth

Call for an appointment or to schedule a curbside pickup of orders; walk-in

Fees

- $100 benefit towards the purchase of prescription glasses every two years for members - 50% down payment is required for eyeglasses prior to ordering. The contact lens must be paid in full prior to ordering. - All costs over benefit amounts will be the responsibility of the patient; vary by service
