Offers the following adult health services to support community members' well-being: - Case Management: Helps people access, coordinate, and monitor needed educational, medical, social, and vocational services based on assessed needs and preferences - Foot Care Clinics: Offers foot care services by appointment, including nail trimming, filing, blood pressure checks, education, and referrals, with no one denied for inability to pay. - Reproductive and Sexual Health: Provides free or low-cost reproductive and sexual health services to all, delivered by nurses and a physician assistant in a confidential setting.
