Eligibility

  • Adults living with a serious mental illness and individuals who are living with a co-occurring substance use disorder- Children and adolescents who have severe emotional disturbance, have been a victim of abuse or neglect, or have been court-ordered for evaluation- Residents of Hennepin County

Provides a multidisciplinary approach for mental health care, offering a range of services. Services offered:- Assessments and Evaluations: Provides diagnostic assessments, psychiatric evaluations, psychological evaluations, and combined psychological and parenting evaluations- Therapy and Education: Provides individual and group therapy, consultations, psycho-education, and wellness for general mental health- Medication Services: On-site pharmacist for medication management and aftercare- Outreach: Provides outreach services to people experiencing homelessness- Whole-person Care: Provides treatment for co-occurring disorders, care coordination, resource assistance, and collaboration with community partners and service providers
