Individuals over the age of 18, meet income guidelines and reside in Blue Earth, Brown, Faribault, Le Sueur, Martin, Nicollet, Sibley, Waseca, and Watonwan counties

Application/registration form Certificate of Rent Paid form dates of birth for each individual listed on tax returns direct deposit information driver's license health insurance forms income forms such as wages from jobs/unemployment IRS letter for Child Tax Credit property tax statement Social Security number Social Security Benefits form year end statements for public benefits
