Youth ages 14 - 22 who have been in a county or Tribal approved out of home placement who live in Chisago, Cottonwood, Isanti, Jackson, Kanabec, Lincoln, Lyon, Murray, Nobles, Pine, Pipestone, Redwood, or Rock counties
