Individuals who are living with a disability who: - Are residents of Blue Earth, Brown, Faribault, LeSueur, Martin, Nicollet, Sibley, Waseca, and Watonwan counties - Need a completed assessment through a county case manager or a public health nurse - Need to have or be eligible for Medical Assistance/Medicaid (MA)
8 am - 4:30 pm Monday - Friday
- Call for more information and to request for a care plan; email - The care plan assessment includes going over the county assessment and organizes information for both the client and the PCA agency to follow. PCA staff works with clients on a care plan before services can be provided.

PCA Choice has been replaced with Community First Services and Supports. The program allows more control of services for the consumer. Services for the Choice Option include: - Activities of daily living - Behavior Interventions: Observes and redirects behavior that interferes with completion of PCA services - Instrumental Activities of Daily Living: Provides meal planning, shopping, light housekeeping, participation in the community
