Eligibility

Individuals residing in Minnesota must meet income guidelines and one of the following conditions: - Diagnosed with a qualifying heart valve condition and receiving treatment - Diagnosed and treated for Acute Respiratory Distress Syndrome (ARDS), Toxic Shock Syndrome (TSS), and/or Sepsis within the last six months - Diagnosed with Merkel Cell Carcinoma (MCC)

?Provides small grants to patients who meet financial and medical criteria on a first-come, first-served basis until funds are depleted. Open funds include: - The Angel Fund: Supports funeral expenses for patients who have passed away from ARDS, TSS, or Sepsis within the last six months. Once approved, this fund provides a one-time $1,000 grant to cover burial or cremation costs. - Heart Valve Financial Aid Fund: Supports patients diagnosed with a qualifying heart valve condition or disorder who are currently receiving treatment, have received treatment within the last six months, or are starting treatment within 60 days. Once approved, the fund offers a one-time $600 grant to help with out-of-pocket expenses related to transportation, lodging, housing, utilities, and food for heart valve treatments. - LEISHLine Financial Aid Fund: Supports patients diagnosed and treated for Acute Respiratory Distress Syndrome (ARDS), Toxic Shock Syndrome (TSS), and/or Sepsis within the past six months. Once approved, this fund provides a one-time $1,000 grant to cover transportation, housing, utilities, or food and nutrition needs. - Merkel Cell Carcinoma Financial Aid Fund: Supports patients diagnosed with Merkel Cell Carcinoma (MCC), a rare skin cancer. Once approved, it offers a one-time assistance of $1,000 to cover transportation expenses related to treatment appointments, lodging, and nutritional needs. Eligible patients include those actively undergoing treatment, treated within the last six months, or scheduled to receive treatment within 60 days.
